Name of patient, age in years, sex, weight in kg, address, mobile/ telephone number.

1. ECG.
2. Echo report.
3. ​Stress Test (T M T)
   
4. Coronary Angiography report.
5. Coronary Angiography Video-clips keeping original video quality intact (from CD).
6. Old Case paper records arranged serially date-wise with page numbering along with any other investigation reports.

 
* Terms and conditions for online consultation/opinion:

1. Online consultation/opinion will be given within 48-72 hours by email only; as a reply to same email address provided to send above information.
2. Online consultation/opinion will be provisional only that to be confirmed after patient clinical examination only.
3. Fees for online consultation /opinion is Rs 500 (Rs five hundred-Indian currency only-INR)
4. Fees to be deposited by NEFT/RTGS/any online money transfer option at following account:                                                            (A/C No: 31920891568, IFSC Code: SBIN 0007430, State Bank Of India, Branch: Dashmesh Nagar-Aurangabad, Maharashtra State.)
5. Email fees transaction details like UTR number/money transfer reference number, date of fees deposit, etc.
6. Free online consultation/ opinion is available only for close family members of all types of  Indian soldiers and Below Poverty Line (BPL) patients provided they should attach its proof attested by any gazetted government officer  (conditions apply).
7. Consultation Refund Policy: if second opinion not given within 48-72 hours; complete fees i.e rs 500 will be refunded to you. For that you have to provide bank account number for money refund/transfer and ifsc code, bank name, branch name, etc.
8. All above patient related information will be archived for ten days only.
9. ​Subject to Aurangabad jurisdiction only.
