 List of information and documents/attachments to be sent by email for second opinion is as follows:

•  Name of patient, age in years, sex, weight in kg, address, email/mobile.
• Echo report
• ECG
• Coronary Angiography report
• Coronary angiography Video-clips (from CD)
• Old Case paper records arranged serially date-wise and with page numbering.

* Terms and conditions for second opinion:

• Fees for second opinion is rs 500=00 (Rs five hundred)
• Fees to be deposited by NEFT/RTGS/any online money transfer option at following account:

• Email fees transaction details like UTR number/amount transfer reference number, date of fees deposit, etc
• Second opinion will be provisional that to be confirmed after patient examination only.
• Second opinion will be given within 48-72 hours by email/sms to email address / mobile number provided by the patient.
• Refund policy: if second opinion not given within 48-72 hours; complete fees rs 500 will be refunded to you. For that you have to provide bank account number and required details for money transfer.
• Subject to Aurangabad jurisdiction only.
